Kelvin Mattocks, 53, and Robert Higgins, 47 were killed on Friday, October October 21 when a trench they were digging on Dartmouth Street filled with water. They were working for Atlantic Drain Service Co.

OSHA has fined the company involved nearly $100,000 in the past, according to a report by Ally Donnelly on NECN. The company has a record of repeat violations for incidents like unqualified daily inspections, not having a rescue team in place for underground work they did in 2015, not properly training employees to recognize hazards, not providing a support system to protect employees in a possible collapse, and not protecticting employees from cave-ins with an adequate protection system.

Federal and state agencies are investigating the incident.
